Your "Official" Name The name that will be displayed to the public next to your panel. If you go by an alias, like "CosplayMan!," you could put that here. Otherwise you can put your name.

Legal Name The name that appears on your government issued ID.

Your Email Email is our primary method of communication, so use an address you will check often.

Your Phone (XXX-XXX-XXXX) In case we need to reach you quickly. Please use the format 555-555-5555.

Type of event. Tell us what kind of event/panel you are submitting.

Discussion: A fan panel or simple Q&A.
Demonstration: Educational lecture or non-interactive how-to or demo.
Workshop: Any hands-on or interactive demonstration.
Participation Game: Something where all, some, or most of the audience will participate.
Other: Everything else. We recommend contacting us first.

Room Type The most common rooms are Theater style. If you can make do with that type of room, we recommend it. Other options will be available on an extremely limited basis for only the best few submissions.

Attendance Limits Unless you have some special reason, we recommend selecting "Any".

Title Official Name of the event as it would appear in a convention program guide.

Short Description - Official Public Description Please write a 2-3 sentence description short enough for our program book.

Full Description - Longer Private Description Please provide a detailed summary to help our staff know where/when best to place you. Focus on the specific details of what you'll be doing during the panel. Only our staff see this description. Write between 3 and 10 sentences. We don't need every last detail, just the basics of what's happening.

Other Contacts Provide the name and phone number for the other people helping with your event.

Technology Needed Our projectors work with most Windows(R) laptops that have a VGA connection. MACs and some really thin laptops require special adapters that we do not provide. A sound system usually comes with 2 to 4 microphones, and we will not accommodate requests for more, you should be able to share them.

Requested Setup Time Most of the time panels get 0 to 5 minutes of setup time, in nearly all cases except for special events. If you request extra time, you might not get it - please try to have your equipment or materials ready to go right away, just in case.

This program is only open to those 18+.
Anyone 12 and under should be supervised by a parent or guardian.
No food or drink is allowed during this program.
